群発頭痛症例　NO４２　飲酒時のみに現れる隠れ群発頭痛

こんにちは。副院長の鈴木です。今回は少し特殊な群発頭痛症例を紹介します。
通常、平均的な群発頭痛の症状としては群発期に入ると１日1～2回発作が起ます。
ですが今回の症例の場合、飲酒さえしなければ発作が出ることはあまりない「隠れ群発頭痛」でした。

３０代　男性　埼玉県
・数年前から発症。右目の奥に激痛。群発期に飲酒すると発作が起きる。ただし必ず出るわけではなく、だいたい７割程度。仕事のお付き合いで飲むことが多いので出ないようにしたい。

・群発期は長いと２カ月続くこともある。飲酒せずに発作が出ることは稀。

［経過］

・施術１回目

今期の発作が始まってから１４日後に来院。普段はあまり首肩こりはない。頭痛の出る右側にはしっかり反応あり。全身を調整していると本人でも気が付かなかった筋肉の硬結があった。

・施術２回目

３日後に来院。２日前に飲酒した際は軽い発作があった。前回よりもさらにしっかり押していく。

・施術３回目

１０日後に来院。先週飲んだ時は出なかったが、今週は飲んだ時と飲まなかったときに１回ずつ軽い発作があった。

・施術４回目

３日後に来院。調子が良く、発作が出ていない。

・施術５回目

１０日後に来院。もう出ていないため群発期終了と思われる。１カ月で群発期を終えられたため精神的にも楽。

今日までたくさんの群発頭痛の方からお話をお伺いしてますが、飲酒はほぼ１００パーセント発作のトリガーになります。もし毎年ある時期に飲酒をすると重めの頭痛に襲われる方は「隠れ群発頭痛」かもしれません。隠れ群発頭痛の方はこれから１年ごとに症状が悪化し、本格的な群発頭痛に変わっていく恐れがあるため症状の軽いうちにしっかり良くしておくのが大切です。

Hello. I am Suzuki, Deputy Director. This time I will introduce some special case of cluster headaches.
Usually, as average symptoms of cluster headache, many patients suffer from seizures once or twice a day when entering the cluster phase.
However, in the case of this case, it was “hidden cluster headache” that seizures would not occur so much unless you had to drink alcohol.

Male in 30’s Saitama
· Onset several years ago. Painful pain in the back of the right eye. Seizures occur when drinking in the cluster. However, it does not necessarily come out, roughly about 70%. Since I often drink with my business association, I want to prevent it from appearing.

· It takes 1 to 2 months to swarm. It is rare that a stroke comes out without drinking alcohol.

Treatment course

· First treatment

I came to the clinic 14 days after my seizure in this season started. I usually have not much neck stiffness. There is a reaction well on the right side where a headache appears. There was hardening of the muscles which the person himself did not notice when adjusting the whole body.

· Second treatment

I came to the hospital after 3 days. When I drunk two days ago there was a mild attack. Treatment pushed further firmly than last time.

· Third treatment

I returned to the hospital after 10 days. I did not go out when I drank last week but there was a mild seizure once in this week when I drank it and did not drink it.

· Fourth treatment

I came to the hospital after 3 days. He is in good form and he does not have a seizure.

· 5th treatment

I returned to the hospital after 10 days. It seems that it is due to end the cluster because it has not already come out. It was psychologically relaxed as the group was finished in one month.

I am talking to you from a lot of clusters patients until today, but drinking will trigger nearly 100 percent seizure. If you drink alcohol at a certain time every year, people who suffer from a heavy headache may be “hidden cluster headache”. There is a risk that symptoms worsen every year from hidden cluster headache, and it may change into full-fledged cluster headache so it is important to treat firmly while the symptoms are light.
