群発頭痛症例　NO３７　３～４年に１度やってくる非常に重い群発期

２０代　男性　埼玉県川口市

・７年前に群発頭痛を発症。今回が３期目。３～４年周期だが群発期の辛さは猛烈。ピーク時はレベル１０の発作が１日に５回起こる。期間は２～３カ月と長い。エネルギーを時間をかけて溜めて一気に放出するタイプ。イミグラン皮下注射を使用すると、発作がすぐ良くなるが副作用で体がものすごくだるくなってしまう。回数も多いのでイミグラン皮下注射の使用は極力避けたい。初回の来院時は群発期に入って１週間程、まだピークを迎える前で発作の回数が１日２回の状態でした。

［経過］

・発作の起こる右ラインのみしっかりと反応有り。全身調整も合わせて行う。来院当日の朝はレベル７が来たので、そろそろピークに入るか？

・５日後に２回目の施術。翌日は発作なし。４日後はまた発作がなかったがそれ以外の日は２回ずつ発作がきた。レベル８程の発作が来はじめたので、状況はあまり良くない。

・２日後に３回目の施術。翌日は２回強い発作が来てしまったが、来院当日は１回のみで弱かった。まだ発作が出ているという点では微妙なところですが、明らかにピークに入るのは防げている。

・３日後に４回目の施術。翌日は発作なしで、２日後は２回来たがいずれもレベル１～２程度。来院当日の１１：００にレベル３が３時間半続き、長かったので辛かった。ピークに入るのが怖すぎてあまり間を空けず連続で施術。明らかに発作のレベルが減少。同時に右ラインの反応も低下。

・２日後に５回目の施術。翌日レベル６×２回が４～５０分。職場のトイレで何とかしのいだ。来院当日の発作はなし。

・６日後に６回目の施術。来院当日まで発作２回のみ３０分程弱い痛みが出た。ご本人もこのまま終わる感覚があったため様子見としました。

次回は半年後に来院頂き、群発のエネルギーを溜めないよう定期調整を行う予定です。

· Cluster headache developed seven years ago. This time is the third term. Although it is 3 to 4 years cycle, the spicyness of the swarm is intense. At peak time level 10 attacks occur five times a day. The period is as long as 2 to 3 months. It collects energy over time and releases it at a stretch. Using Immigran subcutaneous injection, the attack quickly subsides, but the side effect makes the body tremendously lax. Because I have many times, I want to avoid the use of Immigran hypodermic injection as much as possible. At the time of the first visit, it took about 1 week after entering the cluster phase, and the number of seizures was twice a day before reaching the peak yet.

Treatment course

· Only the right line in which a seizure occurs is firmly reacted. Together with whole body adjustment. Since the level 7 came in the morning of the day of arrival, will it be around the peak soon?

· Second treatment after 5 days. There is no seizure the next day. Four days later there was no stroke, but seizures came twice each other day. A level 8 seizure started to come, so the situation is not very good.

· 3rd treatment after 2 days. On the next day, a strong seizure came twice, but on the day of arrival, it was weak only once. Although it is a subtle aspect in that seizures are still coming out, it is obviously preventing it from entering the peak.

· Fourth treatment after 3 days. The next day did not have a seizure, two days later came two times, but level 1 or 2 each. Level 3 lasted 3 and a half hours at 11:00 on the day of visit, so it was painful as it was long. I am afraid to get into the peak and treat it continuously without leaving too much space. Obviously the level of seizure decreased. At the same time the reaction on the right line also declined.

· 5th treatment after 2 days. Next day Level 6 × 2 times is 4 to 50 minutes. I am managed to survive at the workplace toilet. There is no seizure on the day of visit.

· Sixth treatment after 6 days. Until the day of arrival, we experienced weak pain only for 30 minutes after the attacks twice. Because the person himself also had a feeling of ending this way, I made it a witness view.

Next time I will visit the hospital in half a year and plan to make periodic adjustments so as not to accumulate the energy of the cluster.
