群発頭痛症例　NO４４　施術が逆効果！？しかし…（その２）

みなさんこんにちは、副院長の鈴木です。前回NO４３は群発期が長引いたケースでしたが、今回は痛みがいつもより強くなりつつも結果的に早く終息することが出来たケースをご紹介させていただきます。

４０代　男性　神奈川県

・３０代に発症し、その後３年おきに群発期が訪れる。発作が始まると体を動かすだけでも響いて辛いのでじっとしているしかない。毎回１カ月以上続き長い時は２カ月続く。

・薬はイミグラン皮下注射と点鼻を使用したことが過去にあるが、１日の発作回数が増えたり群発期が長引いたりした。

・症状は右頭部。目の奥の激痛、奥歯の痛み。群発期前半はレベル3～5（１０段階中）の痛みがあり、ピーク時期はＭＡＸの痛みに襲われる。

［経過］

・１回目
今期の発作が始まり７日目に来院。まだピーク時期ではないため痛みも最大はない。初回の全身とポイントの施術を行う。

・２回目
４日後に来院。前回施術後にレベル5の痛みが１５分くらいあったが薬使わず今日まで過ごした。前回より更に深めの施術。

・３回目
５日後に来院。３日前からピーク時期に突入。発作が毎回ＭＡＸになり、イミグランを使用。施術は前回と同じ強さで行う。

・４回目
４日後に来院。変化がない。筋肉を押した際の痛みが少なくなってきたため最深部を狙うよう施術。

・５回目
４日後に来院。変化なし。今期の発作はいままでと比べてもひどい気がする。
前回で最大限深く押したため今回はあえてあまりポイントを押さないようにしてみる。
右側の首、肩、大胸筋全てが左に比べかなり硬かったのでそちらをメインに施術。
さらに今回までの反応から『群発期中に施術をすると強い発作が出てくるタイプ』ではないかと考え、一旦少し時間を置いていただくことに。

・６回目
２週間後に来院。先週群発期が終わった。前回あった右側の首から大胸筋にかけての硬さは別人のように減っていた。
結果的に痛みは辛いものであったものの１カ月ちょうどで終えることが出来た。

今回のケースのように施術後症状が軽減せず、むしろ発作が悪化することも稀にあります。
そういった場合でも施術を継続し、原因となっているポイントさえ取り除くことが出来れば群発期の早期終了が望めます。
また、このように群発期中に結果が出た方に関しては、群発期が終わったあとの予防施術により次回の群発頭痛を回避出来る可能性が高いです。
前回の記事でもお伝えしたように一時的に症状が強くなったとしても焦らず施術を続けていく事が重要です。
イミグラン等の薬では群発期の延長や薬害などの懸念がありますので、当院の施術に一歩踏み出していただければと思います。

Treatment course · Treatment The first attack of this phase begins and it is hospitalized on the seventh day. There is no pain at all because it is not peak time yet. Perform initial systemic and point treatment.・ We come to the hospital four days after the second treatment. There was a pain of level 5 for about 15 minutes on the day of the previous treatment, but I spent it without using any medicine. Treatment deeper than the previous time.・ We come to the hospital after the third treatment 5 days. Plunge into peak time from 3 days ago. Seizure is MAX every time and uses imigran. Treatment is done with the same strength as last time.・ The fourth visit after 4 days of treatment. no change. Because the pain when pushing the muscle has decreased, it is treated to aim at the deepest part.
・ We come to the hospital four days after the fifth treatment. No change. Seizures of this term feel worse than ever. I tried to avoid pushing too many points this time because I pressed it as deep as I did last time. The right neck, shoulders, and large pectoral muscles were all relatively hard compared to the left, so we treated them mainly. Furthermore, from the response up to this time, I think that it might be a type in which a strong seizure will come out if treatment is performed during the swarming period, and I will temporarily leave some time.・ The 6th and 2 weeks after the treatment visit. The swarming season ended last week. The hardness from the neck on the right side to the pectoralis major muscle, which was the last time, was reduced like another person. As a result, the pain was painful, but I could finish it in just one month.
As in this case, symptoms do not diminish after treatment, and rather, seizures rarely get worse. Even in such cases, if the treatment can be continued and even the cause point can be removed, the early termination of the swarm period can be expected. In addition, for those who got results in the cluster stage like this, there is a high possibility that the next cluster headache can be avoided by preventive treatment after the cluster stage is over. As I mentioned in the last article, it is important to continue treatment without being diligent even if the symptoms become temporarily stronger. With drugs such as imigran, there are concerns such as the extension of the cluster period and drug damage, so I hope that you take one step in the treatment of our hospital.
