群発頭痛症例　NO４３　施術が逆効果！？しかし…

４０代　男性

６年前に群発頭痛を発症。それ以前から片頭痛と緊張型頭痛に悩まされていた。普段から起こる頭痛の頻度が多く、月に１５回くらい薬を服用するため、薬物乱用性頭痛も併発。

群発期の症状

期間は７～１５日と短い。発作回数は１日１回。持続時間は１～３時間。群発期特有の首コリはあるが、肩のコリ感はなし。左側がメインで、たまに右にも発作が起こる。薬はイミグラン点鼻薬、スマトリプタン、ロキソニンを使用。今期は１０日前よりスタート。群発頭痛の症状だけで考えると一見軽いように思える。

［経過］

・思ったよりもかなり首ラインの反応が弱い。かなりピンポイント的に反応があるので細かく狙う。反応の悪さは、薬物乱用の影響もありそう。過剰刺激には十分注意を払う。

・７日後２回目の施術。２日後にお酒を飲んで発作。それからは朝レベル７位の発作が起きる。スマトリプタン+ロキソニン同時服用で対応。薬の効きは良い。

・２２日後に３回目の施術。前回施術後から発作の頻度が増加。片頭痛と緊張型も混ざる。群発期に左ラインに触れない方が良いと判断。現時点では明らかに群発頭痛の症状を助長している。

・約２か月後に４回目の施術。結局、群発期終了までに２カ月を要してしまった。しかし群発期終了してから調子が非常に良く、普段の頭痛が出ていない。ご本人としては、いままで溜め込んでいた痛みを一気に放出した感覚とのこと。もう反動が出ることはないので、しっかりと施術する。

・２カ月半後５回目の施術。基本的には調子が良く、最近はジムに行き運動も出来ている。１週間程前から右に緊張型頭痛が出ているが軽い。あと数カ月程でやってくるはずの次の群発期を防げればさらに良好。

考察

今症例のように群発期に施術を開始すると、群発期を長引かせてしまうことがごくまれにあります。しかし長い目でみれば改善していきます。ですから最初の段階で一時的に症状が強くなったとしても焦らず施術を続けていく事が、長い目で見るととても重要になります。群発期中はどうしても精神的に辛くなり、今起きている頭痛のことばかりに意識が行ってしまう事は十分理解できます。もちろん群発期中の発作を消し去れれば一番いいことですが、その先の予防や改善に向けての施術にも目を向けて頂けると、明るい未来が創造できるのではないでしょうか。

Cluster headache developed six years ago. I had been suffering from migraine and tension headaches before that. There are many headaches that usually occur, and because they take medicine about 15 times a month, drug abuse headaches also occur.

Cluster stage symptoms

The period is as short as 7 to 15 days. The number of seizures is once a day. Duration is 1 to 3 hours. There is a neck collar specific to the swarm period, but no shoulder shoulder feeling. The left side is the main, and occasionally attacks occur on the right. The drug used imigran nasal spray, sumatriptan, Loxonin. This term starts 10 days ago. It seems seemingly light when considering only the symptoms of cluster headache.

Course of treatment

・ The reaction of the neck line is considerably weaker than expected. Because there is a very pinpoint reaction, I aim at it finely. Poor response may also be due to substance abuse. Pay attention to excessive stimulation.

· Second treatment after 7 days. Two days later I had a drink and had a seizure. From that point, a 7th-place attack occurs in the morning. Corresponds to Sumatriptan + Loxonin coadministration. The effect of the medicine is good.

-The third treatment 22 days later. Frequency of seizures increased since last treatment. Migraine and tension also mix. It is judged that it is better not to touch the left line in the swarm period. At the moment it is clearly promoting cluster headache symptoms.

・ The fourth treatment approximately two months later. After all, it took two months to finish the swarming period. However, after the end of the cluster period, she is in very good shape and has no headache. As the principal, it is a feeling that I released the pain I had accumulated up to now. Since there is no reaction, I will do treatment firmly.

· The second treatment after two and a half months. Basically, I’m in good shape, and recently I am able to go to the gym and do some exercise. I have a tension headache on the right from a week ago, but it is light. It will be better if it prevents the next swarming phase that should come in about several months.

Consideration

If treatment is started in the cluster stage as in the present case, it is very rare that the cluster stage will be prolonged. But in the long run it will improve. So it is very important in the long run that if the symptoms become temporarily strong at the first stage, the treatment will be continued without hesitation. It is understandable that during the swarming period it will be painfully painful and that awareness will only go to the headache that is happening now. Of course, it is best if you can eliminate attacks during the cluster, but if you look at the treatment for prevention and cure beyond that, it is not possible to create a bright future.
