群発頭痛症例　NO４０　終わったと思っていた群発頭痛の再来

あけましておめでとうございます。副院長の鈴木です。
新年最初の群発頭痛症例は、来なくなったと思っていた群発頭痛が６年ぶり発症してしまったパターンです。

40代　男性　埼玉県

・20代後半に発症し、その後非常に強い痛みが２年おきのペースで襲ってきた。
痛みは夜と就寝中１回ずつ、３０分程度続く。群発期間は通常２ヶ月続く。
とある年を境に群発頭痛が来なくなり、もう自分には来ないんだと思っていたところに突然6年ぶりの群発頭痛が始まってしまった。

症状は右頭部。目の奥の激痛、涙が出る、鼻水が出る、目やこめかみが痛む、首肩が張る、発作時以外も目の周りがもやもやする、お酒を飲むと痛みが始まる。
普段から頭痛持ちで筋緊張型頭痛の痛みもあるが群発頭痛の痛みが強烈すぎて気にならないとのこと。
また最近肝臓の調子が悪く、今まではワソラン等の予防薬やイミグランを使用していたが負担を考えるとあまり使用したくない。

［経過］

・施術１回目
今期の発作が始まってから１３日目に来院。久しぶりの痛みだったが強さとしては以前の８０％程度。
頭痛の出る右側にはしっかり反応あり。お仕事の影響で首肩から背中にかけてのコリがかなり強くなっていました。
骨盤の歪みもあったので全身調整と頭痛ポイントの施術を行いました。

・施術２回目
五日後に来院。初回の効果を実感して頂いており、少し楽になったご様子。
全身の硬さはまだまだ抜けきっていないので全身調整もしっかり行いました。

・施術３回目
三日後に来院。１度だけMAXの痛みがあった。イミグランにより抑えることは出来た。
施術に体が慣れてきたこともあり今までよりさらに深く刺激を入れてみました。

・施術４回目
六日後に来院。前回施術後にやはりMAXの痛みがありましたが、その後はかなり落ち着いたご様子。
施術の刺激量を増やしたこともあり反動が少し出てしまったことを考えました。
そのためこの日は調子が良くなっていることも考慮し、前回よりも少ない刺激で様子をみてみることにしました。

・施術５回目
十日後に来院。前回施術後から発作は１度も起こらず、１カ月と普段の半分の期間で終えることが出来たことに感動されていました。
またお薬を使う回数も少なく済んだため、心配されていた肝臓への負担も最小限に抑えることが出来たと思います。

今回の症例では施術していながら痛みが強くなってしまった期間がありました。
実は群発頭痛の方の中にはそういった反応を示す方も少なくないです。
そしてそこを乗り越えて施術を続けて頂いた方々が最終的に良い結果になることがあるのも事実です。
今回の場合も根気強く通っていただき、そして早期終了を勝ち取りました。
また施術が順調に進んだことにより、普段の緊張型頭痛や今後の群発頭痛も予防施術で抑えることが出来るのではないかと思っています。
私としては諦めずに来院していただき、このように良い結果を報告できることに感謝しかありません。

A cluster headache pain case in early New Year is a patient who has developed a cluster headache that I thought it was subsided for the first time in 6 years.

Male in 40’s Saitama

· Onset in the late 20s, followed by a very strong pain on every two years pace.
The pain lasts about 30 minutes, one time at night and at bedtime. Group occurrence period is usually 2 consecutive months.
A cluster headache did not come after a certain year, and suddenly a cluster headache started suddenly in 6 years when I thought that I was cured.

Painful pain in the back of my eyes, tears come out, runny nose, pain in eyes and temple, neck shoulder stretched, around the eyes except when in the creation, pain begins when you drink.
Although I usually have a headache and a pain of muscle tension type headache, it is said that the pain of the cluster headache is too intense and does not mind.
Also recently the condition of the liver is bad, until now I have used prophylactic drugs such as wasolan and imigran, but I do not want to use it when I think of the burden.

Treatment course
· First treatment
I came to the hospital on the 13th day since my seizure in this season started. It was pain after a long time but as strength it was about 80% of the previous one.
Coll from neck shoulder to back was considerably strong by the influence of work. There is a considerable response on the right side where a headache appears.
There was also pelvic distortion so I treated the whole body and treated the headache point.

· Second treatment
I realized the first treatment effect and I felt a little fun.
I have gone through the whole body adjustment as the whole body hardness has not been missed yet.

· Third treatment
Visited 3 days later. I had MAX pain only once. I could suppress it with imigran.
As I got used to the treatment, I tried to stimulate more strongly than ever.

· Fourth treatment
I will be in the hospital after 6 days. After all the previous treatment MAX pain still existed, but after that it was quite calm.
I thought that a little reaction had occurred because I increased the stimulus amount of the treatment.
For that reason, I also decided to take a look at the situation with fewer stimuli than the previous time, considering that the condition is getting better on this day.

· 5th treatment
I will be in the hospital after ten days. I was touched by the fact that since my last treatment I did not have a seizure once, I was able to finish it with a half month as usual.
In addition, because I used few medications, I think that I was able to minimize the burden on the liver I was worried about.

In this case, there was a period in which the pain was intensified although it was not treated.
Actually, if you are seeing a cluster patient, there are not many people who show such reactions.
And it is also true that patients who have overcome this and who have continued treatment may eventually get good results.
The patient went through strongly and strongly, and won the early termination.
I am also thinking that treatment is going smoothly, and normal tension type headache and future cluster headache can be suppressed by preventive treatment.
As I am not giving up, I am coming to the treatment and I am grateful that I can treat and report good results like this.
