群発頭痛症例　NO３０　イミグランの効きにくい、最強クラスの群発頭痛

４０代　男性　川崎市

約３０年前に群発頭痛を発症。最近は２年に１回のペースで群発期がやってくる。イミグラン点鼻薬を使用しているせいか、以前２カ月だった群発期が３～６カ月に伸びている。しかもイミグラン点鼻薬の効きが悪い。発作の回数は１日２回～５回で１～２時間継続する。群発期は発作時以外も頭重感がずっと残る。今群発期は来院される１１日前にスタート。まだピークに入っていない状態。

［経過］

・右ラインに強く反応があったが、発作時ほどの辛さはなし。体全体の緊張強いため、整体で調整。

・５日後に２日目の施術。翌日レベル５（※悠整体院では群発頭痛発作の痛みレベルを１０段階で評価しています。）の群発頭痛発作が来てしまったが、それ以降全く発作なし。これまでの３０年間の苦しみは何だったのかとお喜びの様子。

・１週間後に３回目の施術。前回で群発期終了かと思う程の改善でしたが、この１週間は１日１回発作が来てしまった。しかし普段あまり効かなかった、イミグラン点鼻薬がよく効くようになり、群発頭痛発作が来ても１５分程で和らぐ。完璧ではないもののご本人的には相当楽なご様子。

・１週間後に４回目の施術。来院２日前に３回弱い発作が起きてしまい、昨日は久しぶりに会社を休んでしまった。それまでは調子が良かったため、チョコレートをたくさん食べてしまいぶり返してしまったか？油断してしまったとのこと。今までチョコレートで悪化するような症例は無かったので、私としても驚きでした。片頭痛では聞いたことがありますが、群発頭痛では初めてです。

・６日後に５回目の施術。今日までに３回発作が来た。施術してから時間が経つと群発頭痛が復活してくる。

・１週間後６回目の施術。この１週間はこれまでに比べると調子が悪い。レベル５の群発頭痛発作が１日２回出てしまう。しかしイミグラン点鼻を使えば２０分で治まる。

・１週間後に７回目の施術。この１週間で発作は２回のみレベルも２程度。またここ３日は発作が起きていない。

・８日後に８回目の施術。前回から全く頭痛なし。長い間腰痛持ちということもあり、この日は骨盤矯正と整体で腰痛の施術を中心に行う。

今症例は群発頭痛のなかでもひどい症状でしたが、とても順調に回復しました。少し施術回数が多くなりましたが、男性はとても満足して下さったのでよしとします。結果的に通常３～６カ月続く群発期を１カ月半で終わらせることができました。更にピークを迎えることなく過ごせたのは男性にとっての精神的、肉体的苦痛を相当減らすことができたと思います。悠整体院での施術は１度うまくいけば、その次の群発期もほぼ間違いなく、更に改善したり、群発期を回避することができます。

しかし今回のように症状が重い症例では、改善に時間がかかってしまうことも稀にあります。皆様もご存知の通り群発頭痛は謎めいた部分が多々あります。片頭痛や緊張型頭痛と違い目には見えない不思議なエネルギーががあり、群発期のピークではそのエネルギーが爆発的に暴走し、施術で止めにくくなることがあります。ですから本当は群発期でないときに施術を開始し、群発期を回避するのが理想的と言えます。群発頭痛の特性上、群発期以外はあの痛みを忘れてしまうのでなかなか難しいところですが…

Cluster headache developed about 30 years ago. Recently, the pace of clusters comes at a pace once every two years. Due to the use of Imigran nasal drops, the clustering period which had been two months ago has extended to 3 to 6 months. Besides, imigran nasal medicine is ineffective. The number of attacks lasts from 1 to 2 hours with twice to five times a day. In the cluster period, the sense of weight remains unchanged except at the time of attack. Today, the swarm is started 11 days before going to the hospital. It is not in the peak yet.

Treatment course

· There was a strong reaction on the right line, but there was not as hot as at the time of attack. Because the tension of the whole body is strong, it is adjusted by the body.

· Treatment on day 2 after 5 days. On the next day level 5 (* The pain level of cluster headache attacks is evaluated in 10 levels at Yujitsuin.) Cluster headache attacks have come, but no seizures at all since then. It is a pleasure to see what was suffering in the past 30 years.

· Third treatment after one week. It was an improvement that I thought was whether I had healed last time, but my seizure came once a day for the past week. However, imigran nasal drops worked well, which was not usually effective, and even if cluster headache attacks came, they cured in about 15 minutes. Though it is not perfect, it is quite easy to see him personally.

· Fourth treatment after one week. A weak seizure occurred 3 times before visit and yesterday was absent from work company after a long absence. Did you eat a lot of chocolate and turned back because it was in good condition till then? He told me he had broken off. I had no surprises as I had never experienced a chocolate worse case. I have heard of it in migraine, but it is the first time in a cluster headache.

· Fifth treatment after 6 days. Seizures came three times to date. Cluster headache revives when time passes after treatment.

· The sixth treatment one week later. This week is sick as compared to the past. Cluster headache attack of level 5 comes out twice a day. However imigran point nose can cure in 20 minutes.

· Seventh treatment after one week. In this week, seizures are only two times level 2. Also no seizure has occurred on the last 3 days.

· Eighth treatment after 8 days. There was no headache at all since last time. Sometimes I have a low back pain for a long time, I focus on pelvic orthosis and treatment of low back pain on this day.

The case was a severe symptom among the cluster headache, but it recovered very smoothly. Although the number of treatments increased slightly, patients are very pleased that they are satisfied. As a result we were able to finish the group phase which usually lasts 3 to 6 months in a month and a half. It was able to spend without peaking further, it was able to considerably reduce mental and physical suffering for the patient. The treatment of headache pain at Yujitsuin is better if it goes well once, the next cluster period is also almost certain, further improvement and avoidance of cluster phase can be avoided.

However, in cases with severe symptoms like this time, rarely takes time to improve. As you all know, there are many mysterious parts of cluster headache. Unlike migraine and tension type headache, there is strange energy that can not be seen by the eyes, and in the peak of the cluster period, its energy may explode and runaway, making it difficult to stop by treatment. So it is ideal to start treatment when it is not really in the swarm stage and to avoid the cluster phase.
